Project Request Form 
Purpose
The project request form is used to initiate the review process for IT related projects at Fresno State. The objective of this process is to formalize the steps by which new projects are reviewed, vetted and prioritized. The process will help us in deciding how to evaluate and separate the high priority projects from the lower priority projects.
Approved projects will be aligned with the University's strategic objectives, provide the highest value and will be sustained by Fresno State's infrastructure and technology capabilities.
Requirements
Project requests are required for:
- Projects with more than 80 hours of effort, and/or
- Projects in which the total budget exceeds more than $10,000
If you are not sure if your project meets the requirements listed, please submit your request and the business analyst will contact you within 10 business days.

- The form is intended to provide a good understanding of what the project is and why it should be done
- Depending on the project additional, detailed information may be requested
- The requester will be contacted by the PMO within 10 business days from form submission
